Initial Intake

Service Description

Are you ready to start the journey in reclaiming your health and wellness? As a holistic herbalist, it is my goal to help you reclaim a sense of autonomy and empowerment in your daily self care and long term wellness. During your first visit, we will dive into the concerns stated on your intake form. The areas we will look at include diet, lifestyle, exercise, sleep, water intake, connection to nature, relationships, and herbs. Together, we will create a protocol that will help you rebalance and revitalize your life. This protocol is a team effort, but you are the one with the final say. You will also receive a customized herbal preparation and some take-home information. (all included in the price). After booking this session, you will be emailed the intake form. The more information you can are able to give on your intake the form, the more prepared and present I can be for you during the initial visit. If you have any questions at all, please email me and I will respond as swiftly as I can. I look forward to working with you on this journey. "Pay From The Heart" Pricing Structure - My goal for this type of payment structure is to help make herbal consultations more accessible. •Seed - Covers the overhead for the software needed to offer wellness consultations (online booking, forms, email, etc.) and pays me a fair wage •Sprout - Covers custom formulations and overhead costs, and pays me a fair wage •Growth - Helps "pass it forward" by keeping this payment structure going for those who can't afford a higher fee, covers custom formulations and overhead costs, pays me a fair wage If you'd like to join and the prices above would interfere with your ability to secure food, shelter, or other basic needs, please message me.

Cancellation Policy

I understand that life happens. If possible, please give me as much time notice as possible if you need to cancel or reschedule. Cancellations within 72 hours of your appointment will NOT receive a refund. Cancellations made 72 hours in advance of your appointment will receive a 75% refund, 25% will go to booking fees.
